I am glad to work with you. Once you are scheduled, you’ll receive an email with an intake form.
Please do your best to furnish me with a detailed account of your health history and goals. The intake form is HIPAA compliant, and I am committed to clinical confidentiality.
We’ll begin the session by discussing your current goals for health and wellness, localizing your symptoms, and assessing your relevant function.

MASK POLICY
If you have had any respiratory or immune symptoms in the past 7 days, or any known exposures, please wear a mask.
(I can also supply one.)
The room is well-ventilated, and I will wear a mask during our table work, or at any other time you request.
PAYMENT INFO
I work on a sliding scale of $135-$175/hr. Please select an amount in that range that feels humane and sustainable to you. I don’t presently swipe cards, but I do accept payment via check, cash, Venmo, and Zelle.
I do not bill insurance, but I can provide statements for reimbursement.
LOCATION
The Good Shepherd Center is at 4649 Sunnyside Ave N, Seattle 98103. There is off-street parking.
Suite 300 is one floor up from the main lobby, in the South Wing.
If you need to use restrooms before your visit, they are located to the right of the main lobby.
